In this activity you will review two famous system failures. After reviewing these failures you will write about a personal experience in which you were involved in a system failure. While you are reading these articles think about the limitations of systems--Is there such a thing as a perfect system? If there is always some risk of failure what canwe do to reduce that risk? What have scientists and engineers learned from their past failures? Hindenberg Do you have any recommendations for changing thesesystems?

Discussion Guidelines: System Failure (20 points)

Complete this OR the Systems Thinking assignment. Think about a time in your life when you experienced a system failure. You may use your experience with the nuclear power plant simulator or a different one if you prefer. In your initial post, describe the event in 200 words or less. Read your classmates posts and reply to at least one of them with and idea to solve this problem or avoid it in the future. Each student will:

  • Create an initial post with a description of system failure including what caused the failure. (5 points)
  • State if the failure was due to a subsystem or a total failure of the whole system. State whether or not communication between the system and its parts contributed to the failure. (5 points)
  • Explain what you learned from the experience and what changes you made to avoid the same failure in the future. (5 points)
  • Reply to a classmate suggesting at least two other ways to avoid this failure or improve the system so this failure does not occur.(5 points)

Submit your initial and reply posts to the System Failure Discussion Board before the due date listed on the course calendar.
